Prayer List & Remembrance List Submission Request Form:

If you would like to submit somenoe’s name to the MAF Prayer List or the MAF Remembrance List, please fill out the form below as needed with as much information as possible. Upon the review of MAF staff, the submitted name will be added to the appropriate listing.

Note: It is the policy and practice of MAF to never solicitate any donations from those people whose names appear on the MAF Prayer List. Each name registered on the MAF Prayer List or the MAF Remembrance List will have a MAF Commerative Certificate sent to the address provided above. This service is FREE.

MAF Privacy Policy strickly prohibits any MAF Volunteer from sharing privileged information about those served by MAF with others outside of the MAF organization.

The sponsor of the above named person in need of Prayer hereby authorizes MAF to post information regarding the party’s need for prayer on the MAF website and on FACEBOOK.
